If you’re looking to cancel any membership or service agreement at any Tangible Wellness location, complete this form. Upon submission, you’ll receive a confirmation email and it will be process within 72 hours.

All members or visitors using day passes, free passes or other services at our Fitness Center location are required to fill out this form. This form is provided in person and can also be submitted before your first visit.
